Position Title: Clinical Mentor

Detailed Description of Tasks to be Performed:

Over the course of the program, Epic will engage a variety of Special Experts to amplify local capacity for long-term sustainability and country ownership of key and priority population programming. Special Experts are recognized Indonesian or expatriate TA providers who reside in Indonesia and currently work on HIV issues. These individuals will be placed on flexible consultancy agreements to help the program actualize its demand-driven TA system as well as cultivate evidence-based "push" TA efforts.

 

He/She will work under the direction of the Epic Country Representative and in close coordination with the Epic/Indonesia clinical services team to provide senior-level expertise in clinical services capacity building and health systems strengthening.  He/She will represent Epic at national/provincial level meetings, consultations, and forums, provide input to strategies, agreements, protocols, guidelines, training curricula and suggest improvements to the district-level HIV capacity building structures and systems. Specific activities may include:

 

 

Deliverables/Milestones & Due Dates:

 

1.     Support efforts for TLD program include:

·       Work closely with EPIC clinical unit and PHO/DHO to improve TLD transition implementation at site level meet minimum program requirement and follow national guidelines.

·       Represent EPIC in both National and Provincial TLD /Treatment related meeting and provide input that required by program implementation.

·       Active involvement as trainers in formal training for TLD transition training for health facilities in DKI

·       Support  District mentors to provide district mentorsship related to TLD transition implementation as well as HIV treatment and retention related topic to ensure sustainability of program implementation at site level.

·       Provide clinical talk related to the TLD to the health care services to gain more understanding among the health care providers about the using of TLD.

2.     Support for Test and Start program include.

·       Support District mentors to provide clinical and programmatic district mentorsship regarding to Test and Start Program

·       Provide clinical talk related to Test and Start Program

·       Provide input on Test and Start quality improvement

3.     Support for Viral Load program include.

·       Support District mentors to provide clinical and programmatic district mentorsship regarding to the routine viral load test and unsuppressed viral load result.

·       Provide guidance and expertise on VL test result guideline (sop) for health care services.

·       Provide clinical talk related to the Unsuppressed Viral load result.

·       Provide input on VL related quality improvement

4.     Support for Multiple Month Dispensing include:

·       Support district mentors to provide district mentorsing and clinical district mentorsship to the MMD program in health care service.

·       Provide clinical talk and discussion related to the MMD for the health care providers.

·       Work closely in discussion with National Level, provincial level, and site level on improving MMD implementation along with partners such as PSM if required.

·       Endorsing ART Delivery strategy to health care service and providing TA (within district mentorsship)

5.     Support for Partner Notification program include.

·       Support district mentors to provide clinical district mentoring on Partner Notification program at health care provider.

6.     Support for Advanced HIV Disease and Tuberculosis

·       Support district mentors to provide clinical and programmatic district mentorship regarding to Advance HIV Disease and Tuberculosis

·       Provide clinical district mentorsship for Advance HIV Disease (AHD) and also TB/HIV.

7.     Support for the Lost and Link program include.

·       Support -district mentors to provide clinical and programmatic district mentorship for the Lost and Link program

8.     Support district-level clinical district mentorsship that will include:

·       In close collaboration with EPIC clinical unit and PHO/DHO to provide formal training to clinical district mentors that outlines the roles/responsibilities of a clinical district mentors and orients district mentors on relevant guidelines

·       Support for implementation of mentorship system including provide technical and clinical support for each component of the district mentorship system.

·       During the district mentoring visits, assist district mentors to ensure the good understanding and compliance on current HIV national guidelines (especially the use of background CD4 and routine VL) and strategies actively implemented by EPIC program including Linkage to care, Test & Start, Advance HIV Disease, TB/HIV, TLD transition, Partner Notification, MMS/D, Case management and routine VL coverage also managing for patient with unsuppressed Viral Load result.

·       Provide input for quality improvement of district mentorship system and work closely with EPIC and counterpart to assist on follow up action. 

9.     Facilitate case management discussions that will include:

·       Select and compile interesting cases from WhatsApp consultation or text messaging or phone call and district mentoring session and bring this in quarterly clinical discussion forum/ clinical talk with the participants consist of clinician and CST staff.

·       Provide report related to the process of the quarterly clinical discussion.

10.  In close coordination and collaboration with DHOs, continue to facilitate Pre-service training for new rotated staff in DKI  that include:

·       Update basic HIV related knowledge or guideline for affected health staff.

·       Monitor the improvement of affected staff that ensure they will be able to carry basic HIV services task in health facility.

·       Provide report to DHOs including EPIC and suggestion as follow up of pre-services training for affected staff.

11.  Prepare clinical district mentorship job aides that distill clinical district mentorship guidelines or quality of care SOP(s) into an easy-to-read or use format.  E.g. how to prepare a client for ART initiation; how to read and interpret viral load testing results; what to do in case of treatment failure. 

12.  Prepare a listing of key documents – guidelines, SOPs – that doctors or nurses need to know for the delivery of quality testing and treatment (that could be placed in a virtual resource or app).

13.  Provide guidance, input, and technical support (soft skill to raise scholarship, networking to any relevant HIV program supported society, motiv8 etc.)  in the development of health facilities motivation improvement strategy

14.  Provide technical input on Test and Start TA Packages development and support on implementation in health facility.

15.  Work with EPIC clinical services unit, MoH, PHO, DHO and any relevant stakeholder to build strong networking between clinician in the term of HIV clinical consultation and clinical service referral

16.  Participate in HIV meeting at the national, provincial or district level and provide any necessary technical input.

17.  Participate in any EPIC activity as requested and approved by the Technical Monitor.

Provide summary of monthly report include copies of any TA products to Clinical Services Unit Technical Supervisor's and provide update on:

  • Summary activities conducted for supporting TLD Transition plan at facility level, provincial or National level
  • Summary on supporting clinical district mentorsing system report that include issues elaborated and improvement plan, TA plan and on site TA result that reported through current reporting format

The 30th every month during service period

Report any related product created including PPT, draft of document including any SOP, Training materials as supporting document of TA conducted

 

The 30th every last month of the quarter
